All Students enrolled in A+ must read, sign, and have their parents/guardian sign the following contract.

 

A+ Student Orientation Form 2013-2014

 

Please read all the information provided with a parent or guardian and sign and date the

document. The signed form needs to be returned to the teacher within 2 weeks of enrollment or the student will be locked out of A+ until the signed form is returned.

 

A+ Policy:

  • Students should initial at the end of each line and sign at the end of the document to indicate that they have read and understand the policies.

 

1. Students must earn a minimum of 60% on the Final Exam in order to earn credit for the course. Students will have a maximum of three attempts to earn a 60% on the Final Exam.  lf the student does not pass the Final Exam after the third attempt, the student will receive an F in the course. _______ (initial)

 

2.  Students must complete the course within the semester.  If the course is not complete by the end of the semester, the student will earn an “F”.  _______ (initial)

 

3. A+ courses are not accepted by the NCAA Clearinghouse as core credit for college athletics. _______ (initial)

 

4. A+ courses may not be accepted by colleges and universities.  Students and parents should contact postsecondary institutions or branches of the military service prior to taking a course to determine if the course will be accepted for admission. ______ (initial)

 

5. Students will be allowed to work on A+ from home, however, the Assessment A, Assessment B, and Final Exam must be administered in person and monitored at the school. _______ (initial)

 

6. Students must complete 100% of the required work and pass the Final Exam with a 60% or better to be eligible to receive credit in an A+ class. _______ (initial)

 

7. For students taking A+ outside of school as a distance education course, there is a  student expectation of making progress or two way communication, at a minimum of once a week, between the student and the teacher. _______ (initial)

 

8. All WCSD attendance policies apply to your A+ class. _______ (initial)

 

9. Students will earn a letter grade (A, B, C, D, F or INC) in A+.  NM (No Mark) is not an option for a grade in A+. Students who receive an INC will have 3 weeks into the next available term to complete their coursework or it turns into an F.  _______ (initial)

 

10. A + courses cannot be used to improve a previously taken class in which the student received a D- or better.  Courses can replace a previously taken class in which the student received an F. _______ (initial)

 

11.  Students will be allowed to use hand-written, teacher approved notes on the Final Exam.  Students may be allowed to use typed notes on the Final Exam with teacher approval.  Students will not be allowed to use Internet sites, screen shots, pictures on electronic devices, copied and pasted notes, or notes from other students. It is very important the student takes good notes throughout the course.  _______ (initial)

 

12.  A+ students will be allowed to access only A+ curriculum while working in the lab.  No other internet usage will be allowed without permission.  _______ (initial)

 

13.  Intentional damage to the computers is the fiscal responsibility of the student and parent.  Do not tamper with the computers, control panel, keyboard or mouse.  Do not load any programs on to the computers.   _______ (initial)

 

14.  Calculator use for A+ math is allowed for Geometry 1 level classes and above.  Students on an IEP may have an exception to this WCSD rule. _______ (initial) 

 

15.  Parents and students are also responsible for ensuring the student is enrolled in the correct subject on A+.  Please review your transcript and communicate with your counselor and teacher if you feel you are in an incorrect subject. _______ (initial)

 

16.  Cell phone use in the lab is not allowed at any time.  This includes placing or taking calls, texting, or using the internet on your phone.  The teacher has discretion to allow students to quietly listen to music through your phone, but never during Assessments or Final Exams. _______ (initial)

 

17.  The A+ lab is not a study hall.  Assignments from other classes are not to be worked on during A+ class until all assigned courses are completed. _______ (initial)

 

18.  Students must abide by the honor code and submit work that is original and created by the student.  Students who do not will face consequences ranging from resubmission to removal from the course.  _______ (initial)

 

19.  A+ uses the same 90/80/70/60 grading scale as other WCSD courses. _______ (initial)

 

20.  It is possible to earn more than .5 credit per semester if the student meets the requirements from above for more than 1 subject in a semester.  _______ (initial)
